Jardiance for type 2 diabetes: how empagliflozin works and what its trials found

Jardiance is empagliflozin, a once-daily SGLT2 inhibitor tablet, not insulin. It lowers A1C by roughly 0.7 to 0.9 points against placebo, and its heart and kidney trials earned it approvals for heart failure and chronic kidney disease as well as diabetes.

Jardiance is the brand name of empagliflozin, a tablet in the SGLT2 inhibitor class. It is not insulin and does not work through insulin: it makes the kidneys pass extra glucose into the urine [1]. In its first monotherapy trial, Jardiance for type 2 diabetes lowered A1C by 0.74 points at 10 mg and 0.85 points at 25 mg more than placebo over 24 weeks [2]. Its FDA label also covers reducing the risk of cardiovascular death in type 2 diabetes with heart disease, and uses in heart failure and chronic kidney disease, based on four large outcome trials [1].

Is Jardiance insulin? What the SGLT2 class does

No. Insulin is a hormone given by injection; Jardiance is a tablet that acts on the kidney. Normally the kidney filters glucose out of the blood and then takes almost all of it back through a transporter called SGLT2 (sodium-glucose cotransporter 2). Empagliflozin blocks that transporter, so more glucose leaves in the urine and blood glucose falls [1]. Because the effect does not depend on insulin, Jardiance on its own rarely causes low blood sugar; the risk rises when it is combined with insulin or a sulfonylurea [1].

The FDA label lists four uses [1]:

  • improving glycemic control, alongside diet and exercise, in adults and children aged 10 and older with type 2 diabetes;
  • reducing the risk of cardiovascular death in adults with type 2 diabetes and established cardiovascular disease;
  • reducing the risk of cardiovascular death and hospitalisation for heart failure in adults with heart failure;
  • reducing the risk of kidney function decline, kidney failure, cardiovascular death and hospitalisation in adults with chronic kidney disease at risk of progression.

It is not recommended in type 1 diabetes, where it may raise the risk of ketoacidosis [1].

How much Jardiance lowers A1C

In the 24-week monotherapy trial in people new to drug treatment, A1C fell 0.74 points more than placebo with 10 mg and 0.85 points more with 25 mg; sitagliptin, the active comparator, lowered it by 0.73 points [2]. Those are the doses the label states: 10 mg once daily, which may be raised to 25 mg for glycemic control [1].

The ADA Standards of Care 2026 grade SGLT2 inhibitors as intermediate to high in glucose-lowering efficacy, below GLP-1 receptor agonists, tirzepatide and insulin [3]. Jardiance is mainly chosen for what it does beyond A1C. The ADA recommends an SGLT2 inhibitor with proven benefit for people with heart failure or chronic kidney disease, independent of their A1C [3]. Its glucose-lowering effect also depends on kidney function, since it works through the kidney, and the label sets kidney-function limits for each use [1].

EMPA-REG OUTCOME and the heart and kidney trials

TrialWho took partMedian follow-upPrimary result, empagliflozin vs placebo
EMPA-REG OUTCOME7,020 adults with type 2 diabetes and high cardiovascular risk3.1 yearsHeart attack, stroke or cardiovascular death: 10.5% vs 12.1%, hazard ratio 0.86 [4]
EMPEROR-Reduced3,730 adults with heart failure and reduced ejection fraction, with or without diabetes16 monthsCardiovascular death or heart-failure hospitalisation: 19.4% vs 24.7%, hazard ratio 0.75 [5]
EMPEROR-Preserved5,988 adults with heart failure and preserved ejection fraction, with or without diabetes26.2 monthsSame outcome: 13.8% vs 17.1%, hazard ratio 0.79 [6]
EMPA-KIDNEY6,609 adults with chronic kidney disease at risk of progression2.0 yearsKidney disease progression or cardiovascular death: 13.1% vs 16.9%, hazard ratio 0.72 [7]

EMPA-REG OUTCOME was the trial that changed how SGLT2 inhibitors were viewed. Heart attack and stroke rates did not differ significantly, but empagliflozin lowered cardiovascular death from 5.9% to 3.7% (a 38% relative reduction), heart-failure hospitalisation from 4.1% to 2.7% (35%) and death from any cause from 8.3% to 5.7% (32%) [4]. The two EMPEROR trials then showed benefit in heart failure regardless of diabetes [5][6], and EMPA-KIDNEY did the same for chronic kidney disease [7]. Across 453 trials of glucose-lowering drugs, SGLT2 inhibitors as a class reduced heart-failure hospitalisation and end-stage kidney disease [8].

Jardiance side effects and label warnings

The most common adverse reactions on the label are urinary tract infections and genital yeast infections [1]. In EMPA-REG OUTCOME, genital infections were the one adverse event clearly increased with empagliflozin [4]. The label's warnings cover [1]:

  • ketoacidosis, which can occur with glucose levels that are not very high;
  • volume depletion and low blood pressure, from the extra fluid lost in urine;
  • serious urinary infections, including urosepsis and pyelonephritis;
  • hypoglycemia when combined with insulin or an insulin secretagogue;
  • necrotizing fasciitis of the perineum (Fournier's gangrene), which is rare;
  • genital mycotic infections.

The ADA Standards list the same class risks, with genital mycotic infections and diabetic ketoacidosis named explicitly [3].

What does a Jardiance pill look like?

According to the prescribing information, both strengths are pale yellow, film-coated, biconvex tablets [1]:

  • 10 mg: round, debossed with "S10" on one side and the Boehringer Ingelheim company symbol on the other.
  • 25 mg: oval, debossed with "S25" on one side and the company symbol on the other.

Combination tablets that contain empagliflozin with metformin or linagliptin are separate products with their own appearance, so the markings above apply to Jardiance alone.

The comparison is indirect: no trial has tested Jardiance against semaglutide or CagriSema, and the A1C figures come from different trials with different populations. What the ADA uses to separate them is not A1C but the other conditions: SGLT2 inhibitors for heart failure and kidney disease, GLP-1 receptor agonists and tirzepatide when weight management is a goal [3]. The two classes are often combined.

CagriSema adds the amylin analogue cagrilintide to semaglutide. In REIMAGINE 2 its extra A1C effect over semaglutide alone was 0.16 points [10]; in REIMAGINE 1, in people on diet and exercise alone, A1C fell 1.8 points against 0.1 on placebo [12]. The semaglutide record and the cagrilintide vs semaglutide comparison carry the detail.
