Survodutide dosage calculator
Enter a vial and a dose and this returns the concentration, the volume and the syringe units. Underneath it, the same arithmetic worked out in advance for common vials, and the doses published studies of Survodutide actually used, every one quoted and cited. It converts numbers. It does not suggest what to take.
Work out concentration and what to draw
Two divisions, both shown. Concentration = mass in the vial ÷ diluent volume. Volume to draw = mass you want ÷ that concentration. Syringe units are that volume × the units per millilitre on your syringe's own scale.
Worked example: 5 mg in 2 mL is 2,500 mcg/mL, so 250 mcg is 0.1 mL, which is 10 units on a U-100 syringe and 4 units on a U-40. The scale is printed on the barrel; reading a U-40 syringe as if it were U-100 is a two-and-a-half-fold error.
Survodutide reconstitution chart
Units on a standard U-100 insulin syringe, where 100 units is 1 mL. The rows are the absolute doses quoted in studies of Survodutide, from 1.8 mg to 6 mg, so this chart is specific to this compound rather than a generic ladder. More diluent means more units for the same dose, which is easier to measure accurately and empties the vial no faster.
| Dose | 1 mL diluent (2,000 mcg/mL) | 2 mL diluent (1,000 mcg/mL) | 3 mL diluent (667 mcg/mL) |
|---|---|---|---|
| 1.8 mg | 90 | 180 >1 mL | 270 >1 mL |
| 2.7 mg | 135 >1 mL | 270 >1 mL | 405 >1 mL |
| 3.6 mg | 180 >1 mL | 360 >1 mL | 540 >1 mL |
| 6 mg | 300 >1 mL | 600 >1 mL | 900 >1 mL |
| Dose | 1 mL diluent (5,000 mcg/mL) | 2 mL diluent (2,500 mcg/mL) | 3 mL diluent (1,667 mcg/mL) |
|---|---|---|---|
| 1.8 mg | 36 | 72 | 108 >1 mL |
| 2.7 mg | 54 | 108 >1 mL | 162 >1 mL |
| 3.6 mg | 72 | 144 >1 mL | 216 >1 mL |
| 6 mg | 120 >1 mL | 240 >1 mL | 360 >1 mL |
| Dose | 1 mL diluent (10,000 mcg/mL) | 2 mL diluent (5,000 mcg/mL) | 3 mL diluent (3,333 mcg/mL) |
|---|---|---|---|
| 1.8 mg | 18 | 36 | 54 |
| 2.7 mg | 27 | 54 | 81 |
| 3.6 mg | 36 | 72 | 108 >1 mL |
| 6 mg | 60 | 120 >1 mL | 180 >1 mL |
Read a figure off this chart only if it matches the vial in front of you. A vial labelled a different strength, or reconstituted with a different volume, gives a different number for the same dose, and that is the single most common way these calculations go wrong.
Doses in published studies of Survodutide
Each row is quoted from a paper in this compound's source ledger and carries the kind of evidence it came from. 4 of 4 come from human studies; the rest are animal or laboratory work.
| Evidence | Dose as the paper states it | Source |
|---|---|---|
| Human clinical trial | In SYNCHRONIZE-1, 725 adults with obesity received once-weekly survodutide titrated to 3.6 mg or 6.0 mg, or placebo, for 76 weeks. | pmid-42253238 |
| Human clinical trial | In SYNCHRONIZE-MASLD, 216 adults with obesity and at-risk liver disease received 6.0 mg weekly or placebo, two to one. | pmid-42252333 |
| Human clinical trial | The type 2 diabetes phase 2 tested 0.3 to 2.7 mg once weekly and 1.2 or 1.8 mg twice weekly for 16 weeks against placebo and open-label semaglutide. | pmid-38095657 |
| Human clinical trial | The phase 3 programme titrates to 3.6 or 6.0 mg with dose flexibility permitted. | pmid-39495965 |
Escalation schedules used in studies
Where a trial raised the dose over time rather than starting at the target, this is what it did. The starting dose is often the part that decides tolerability, which is why it is reported here and not only the dose the trial finished on.
- Randomized controlled trial, humans, n=146, 2026: The most frequently reported adverse events with survodutide were gastrointestinal, commonly occurring during dose escalation, and were generally of mild-to-moderate severity. pmid-42252333
- Randomized controlled trial, humans, 2026: Participants aged ≥18 years with a body mass index (BMI) ≥27 kg/m 2 and T2D were randomized 1:1:1 to weekly subcutaneous survodutide (up-titrated to 3.6 or 6.0 mg) or placebo with recommendations for modified diet and physical activity. pmid-41216778
- Randomized controlled trial, humans, n=725, 2026: Participants aged ≥18 years with BMI ≥30 or ≥27 kg/m 2 with ≥1 obesity complication without T2D were randomized 1:1:1 to double-blind, once-weekly, subcutaneous injections of survodutide (up-titrated to 3.6 or 6.0 mg) or placebo for 76 weeks. pmid-41187967
- Randomized controlled trial, humans, n=387, 2025: Totally 387 people (aged 18-75 years, BMI ≥27 kg/m 2 , without diabetes) were randomized 1:1:1:1:1 to once-weekly subcutaneous survodutide (0.6, 2.4, 3.6 or 4.8 mg) or placebo for 46 weeks (20-week dose escalation; 26-week dose maintenance). pmid-39821928
- Phase 1 clinical trial, humans, n=24, 2023: A phase Ib study (NCT03591718) investigated multiple rising doses (MRDs) of BI 456906 (escalated over 6 [Part A] or 16 [Part B] weeks) in 125 adults with a BMI of 27-40 kg/m 2 . pmid-36527386
How long studies ran, and how far a vial goes
A vial lasts for a number of draws, not a number of weeks: divide the mass in the vial by the mass per draw and round down, which the vial planner does. How long that represents depends on frequency. For context, these are the durations studies of Survodutide actually ran for.
- Randomized controlled trial, humans, n=413, 2026: durations stated: 16 weeks; 46 weeks. Trial 1404-0002 randomised 413 participants with type 2 diabetes on metformin to receive survodutide, placebo, or semaglutide over 16 weeks. pmid-42331726
- Randomized controlled trial, humans, n=387, 2025: durations stated: 46 weeks. After 46 weeks of survodutide treatment, females had greater reductions in bodyweight and waist circumference than males. pmid-39821928
- Randomized controlled trial, humans, n=50, 2024: durations stated: 75 years; 16 weeks. This Phase II, multicentre, randomised, double-blind, parallel-group, placebo-controlled study, conducted in clinical research centres, assessed survodutide in participants aged 18-75 years with type 2 diabetes, an HbA 1c level of 53-86 mmol/mol… pmid-38095657
How Survodutide was given
The route changes what a dose means. The same number given intravenously and subcutaneously is not the same exposure.
- Randomized controlled trial, humans, n=725, 2026: administration by subcutaneous route reported. In this phase 3, double-blind trial, we randomly assigned adults with a body-mass index (BMI; the weight in kilograms divided by the square of the height in meters) of 30 or higher, or 27 or higher with at least one obesity-related… pmid-42253238
- Randomized controlled trial, humans, n=146, 2026: administration by subcutaneous route reported. Participants were randomized (2:1) and treated with once-weekly subcutaneous injections of survodutide 6.0 mg (n = 146) or placebo (n = 70). pmid-42252333
- Randomized controlled trial, humans, 2026: administration by subcutaneous route reported. Participants aged ≥18 years with a body mass index (BMI) ≥27 kg/m 2 and T2D were randomized 1:1:1 to weekly subcutaneous survodutide (up-titrated to 3.6 or 6.0 mg) or placebo with recommendations for modified diet and physical activity. pmid-41216778
- Randomized controlled trial, humans, n=725, 2026: administration by subcutaneous route reported. Participants aged ≥18 years with BMI ≥30 or ≥27 kg/m 2 with ≥1 obesity complication without T2D were randomized 1:1:1 to double-blind, once-weekly, subcutaneous injections of survodutide (up-titrated to 3.6 or 6.0 mg) or placebo for 76 weeks. pmid-41187967
- Randomized controlled trial, humans, n=387, 2025: administration by subcutaneous route reported. Totally 387 people (aged 18-75 years, BMI ≥27 kg/m 2 , without diabetes) were randomized 1:1:1:1:1 to once-weekly subcutaneous survodutide (0.6, 2.4, 3.6 or 4.8 mg) or placebo for 46 weeks (20-week dose escalation; 26-week dose maintenance). pmid-39821928
- Randomized controlled trial, humans, n=50, 2024: administration by subcutaneous route reported. Aims/hypothesis The aim of this study was to assess the dose-response effects of the subcutaneous glucagon receptor/glucagon-like peptide-1 receptor dual agonist survodutide (BI 456906) on HbA 1c levels and bodyweight reduction. pmid-38095657
Questions people ask
- How many units is 1.8 mg of Survodutide?
- It depends entirely on how the vial was reconstituted. In a 5 mg vial with 2 mL of diluent it is 72 units on a U-100 syringe; with 1 mL it is 36. The chart above gives the rest.
- Does adding more diluent waste the peptide?
- No. The mass in the vial does not change. More diluent spreads the same mass through more liquid, so each dose is a larger, easier-to-measure volume and the vial yields exactly as many doses as before.
- Why does this page not recommend a dose?
- Because a dose is a clinical decision rather than a division. It depends on why someone is taking something, what else they take and how their kidneys and liver are working, none of which is knowable from a number typed into a box.
- Where do the doses in the table come from?
- Each is quoted from a specific paper indexed in Europe PMC, linked in the source column, and labelled with the kind of study it came from. The full evidence is on the Survodutide record.
What this calculator will not do
It will not take a body weight and return a milligram figure, and it will not tell you how often to draw. The table above is the published record, not a menu. A dose that appeared in a trial appeared under supervision, with monitoring, in people selected for that trial.
